GLP-1 Maintenance Dose Guide

So, you’ve reached your goal weight using GLP-1 medications and a lot of hard work. Now what? As a Family Nurse Practitioner specializing in weight loss, here's how I guide my patients into maintenance.

1. Long-Term Maintenance

From the beginning, I inform my patients that if they’ve struggled with their weight for many years, they will likely need a low-maintenance dose long-term. Fortunately, GLP-1 medications remain effective and safe with extended use.

2. Stabilize Before Reducing Dose

Once patients reach their goal weight, I advise them not to change anything for several weeks. They should continue their current dose of semaglutide or tirzepatide, maintain healthy eating habits, stay hydrated, and keep up with their exercise routines. This allows their bodies to settle into the new weight.

3. Gradual Dose Reduction

After stabilizing, I suggest a very slow reduction of their GLP-1 dose by 5 units every week or two, allowing their appetite to gradually return. Depending on their response, they may completely transition off the medication, or, like many of my patients, prefer to stay on a small dose taken every 7-10 days. I typically recommend taking some medication at least every 10 days to avoid it completely leaving their system. For most patients, the maintenance dose is usually 10-15 units every 7-10 days.

4. Maintain Healthy Habits

The main goal during this phase is to keep appetite under control and maintain the healthy eating habits developed while on the medication.
